What's it like to be a Case Manager?
A Case Manager (Kaiwhakahaere Kēhi) is a role centred on coordinating and providing services to individuals in need, such as individuals receiving healthcare or those involved in the social care system. Responsibilities for Case Managers range from developing, implementing and monitoring a person-centred care or support plan. They act as an intermediate between clients and various institutions to ensure the client receives appropriate services. Typically, Case Managers work in sectors such as healthcare, social work, mental health services and community programs.
Tasks and duties
- Assessing and evaluating clients' needs and situations to develop appropriate care plans.
- Coordinating and providing care that is safe, timely, effective and client-centered.
- Handling case assignments, draft service plans, reviewing progress and determining case closure.
- Facilitating multiple care aspects like medication and health education.
- Maintaining comprehensive case history records and reporting in a timely manner.
- Coordinating with insurance companies for appropriate coverage and reimbursement.
- Collaborating closely with healthcare professionals involved in the day-to-day care of clients.
